On top, there is a nut, with a plastic cover. There is nothing between the fork crown and the head tube other than a thin plastic ring. However, the bearings are inside the frame. I have a bike with a clearly threaded headset, with a quill stem inserted into it. The article seems to state that only threadless headsets can be internal.
